Good to Know
Texas Children's Health Plan (TCHP)
Accepted for clinic visits, but our affiliated Memorial Hermann delivery hospitals are not in-network with TCHP for delivery. OB patients on TCHP need to switch to CHC Medicaid at least 60 days before their due date.
Humana HMO plans
Require a referral from your primary care provider before we can schedule your visit.
Benefits are verified day-of
Coverage can change daily, so we verify your benefits the day of your visit rather than beforehand. We can't quote an exact copay over the phone, but we'll walk you through your costs before you're seen.
One plan at a time
We're unable to bill private insurance and Medicaid/Medicare together for the same visit — if you have both, the private plan needs to be dropped first.

We're out-of-network — self-pay is available.
Molina Healthcare (Private and Medicaid)
We're out-of-network for private plans (self-pay available); for Medicaid, ask your plan about switching to an in-network option.
Aetna HMO plans
Aetna PPO/POS plans are accepted — HMO specifically is not. We'll confirm your plan type when we verify benefits.
BCBS with a Member ID starting with T2G
BCBS is accepted generally — this specific prefix is the one exclusion.
UHC / United Healthcare (Private and Medicaid)
Not accepted at our facility. For Medicaid, ask your plan about switching to an in-network option.
UMR
Not accepted at our facility.
Community Ultra Select Bronze 016 (marketplace plan)
We're out-of-network for this specific plan.
